Community Day – Wednesday, October 16, 2019
We've made Registration Easy! There are three ways to pre-register for attendance on Community Day.
1. Pre-Register On-Line: If you have an e-mail address, you may pre-register on-line.
Online registration is completed in two steps.
Step 1: Click on the "Continue to Registration" button below. You will be redirected to another page where you can register for the event.
Step 2: We will be providing a free lunch to the first 25 registrants. You can preview the menu here. Afterwards, simply select from the menu on the registration form. After the first 25 spots are filled, a lunch menu will be available for you to order from.
2. Pre-Registration by Phone: Call our office at (870) 425-2460 and a member of our Team will assist you in the registration process. Our office hours are from 9 to 5 on Monday through Thursday and 9 to Noon on Fridays.
3. Pre-Registration by Mail: Print out the registration form and mail it to us at the following address:

Remember to enclose a check payable to Elder Care Symposium if you wish to pre-order your lunch. The menu and prices are posted here. We will only pre-order meals which we have received payment for on or before October 15th, 2019. The first 25 registrants will receive a free meal!
Fees to Attend? None. This event is brought to you by our valued sponsors. Please take the time to visit with them before and after the conference and during breaks. Remember to thank them for their support.
Why Pre-Register? Planning a conference without knowing the number of attendees is difficult at best. Pre-registration allows us to plan efficiently and to avoid waste. This in turn keeps our costs in line allowing us to continue presenting the Symposium to our community without charge.
For example, pre-registration information is used to determine:
How many information packets a speaker/presenter should prepare to hand out to attendees;
The quantity of food and beverage to have available for attendees during the breaks in between sessions; and
How many chairs to set up in each room and which room to use to accommodate attendees.
We also use the pre-registration process to allow attendees to pre-order lunch from our limited menu and to apply for a scholarship (limited quantities available) which will pay for adult day care services for the attendee's loved one, freeing the attendee to participate in the Symposium.
Will my information be kept Confidential? Yes. We have a secured site designed to protect your registration information. We also will not SELL or RELEASE your information to third parties. The information obtained will be used primarily for planning purposes. We will also use the information to notify you of any changes to our plans and to confirm your attendance.

Walk-in Registration: Attendees may register in person on the morning of the event. However, lunch may not be available for purchase at that time.
Losses: The sponsors will not be responsible for losses incurred by registrants.
Cancellation: The sponsors further reserve the right to change locations if deemed necessary.
